Basic description
Subacute or transition care nurses care for patients who have skilled care needs after an acute illness or hospitalization. Care could be rendered in a rehabilitation or skilled nursing facility or in a designated unit of a health care facility.
Educational requirements
Registered nurse preparation and Basic Life Support certification are required.
Core competencies/skills needed
â– Medical and surgical nursing experience
â– Excellent assessment skills, and skill in the care of patients on intravenous therapy, ventilator, or tracheostomy
â– Strong computer and documentation skills
â– Ability to solve staffing difficulties when they arise
â– Ability to work in a dynamic unit where patient admission and discharges are high
